Is it being actively used by threat actors? If the answer is no, its priority drops dramatically.

It’s crucial to ask what the actual damage to the business would be if a specific vulnerability were to be exploited. For example, a remote code execution flaw in a public-facing application handling sensitive data is an existential threat. The exact same flaw in an internal administrative tool is, at worst, an inconvenience. In the end, you can’t know the real threat without first knowing what you’re trying to protect.

The critical first step in the industry is to map these automated findings to an application’s actual runtime environment. This process helps define the real attack surface. For instance, when a scanner flags a vulnerability in an open-source library, the immediate question is not about its CVSS score, but whether that specific vulnerable function is even used by the application in production. Context is everything.

Step 2: Asking the Right Questions

With proper context, security professionals can cut through the noise by applying a filter of targeted questions to any given vulnerability. This simple interrogation can often disqualify the vast majority of alerts from needing immediate attention. The questions typically revolve around real-world risk factors:

  • Does the flagged code actually run in a production environment?
  • Is the specific code reachable from the internet or by an untrusted user?
  • Does a proven, public exploit for this vulnerability exist right now?
  • If this were to be exploited, what systems, data, or business functions would actually be compromised?

Every “No” to these questions dramatically lowers the real-world risk, pushing it further down the priority list. This is how professionals systematically dismantle the mountain of noise to find the handful of threats that genuinely demand immediate attention.

Step 3: Unifying Data for a Complete Picture

One of the biggest blockers to effective prioritization is that security data is often siloed.
